

What is a Well-being Plan?
Individual planning represents the first step in an orderly process to accomplish what is necessary to improve your well-being or overall health status. The planning and evaluation cycle, allows us to be successful in managing health problems as efforts to understand and approach them in an orderly fashion are fundamental to well-being.

At Metabolism you will receive a written Well-being Plan based on your individual health needs which supports you to reach your health goals by:

providing an organised framework of options based on your signs/symptoms/test results etc.;

allocating and managing resources;

outlining agreed goals to reach a positive sense of well-being;

being a vehicle to monitor goals and progress;

considering possible future needs;

providing information to and liaising with, other health practitioners which may be involved in your well-being goals.

The Well-being Plan is provided as part of the consulting process and is at no extra cost.
